Reimagine Care came to us with a clear and meaningful goal: how might we improve oncology patient outcomes following treatment sessions, while easing the burden on traditional care settings? Through a collaborative workshop, we explored remote-first care delivery models and co-created a personalized AI chat experience that connected cancer patients with licensed practitioners using Apple Messages for Business, Google Business Messages, and a private LLM to provide real-time triage support from the comfort of the patient's home.
